What is Lo Loestrin FE?

Lo Loestrin Fe is a low-dose combination birth control pill (oral contraceptive) containing two different female hormones. It helps to prevent pregnancy.

Lo Loestrin Fe contains an estrogen called ethinyl estradiol and a progestin called norethindrone acetate. Lo Loestrin Fe works primarily by suppressing ovulation, which is when an egg is release from one of your ovaries each month. Combined birth control pills, such as this one, also work by thickening the mucus at the neck of the uterus (womb). This makes it harder for sperm to get through and reach an egg. They also thin the lining of the uterus, which reduces the chances of a fertilized egg implanting and growing.

Lo Loestrin Fe was approved for use by the US Food and Drug Administration (FDA) in 2010. A generic version of Lo Loestrin Fe has not been approved.

What is Lo Loestrin FE used for?

Lo Loestrin Fe is a prescription medication used by women who want to prevent pregnancy.

Important information

Warning To Women Who Smoke

Do not use Lo Loestrin Fe if you smoke cigarettes and are over 35 years old. Smoking increases your risk of serious cardiovascular side effects (heart and blood vessel problems) from birth control pills, including death from heart attack, blood clots or stroke. This risk increases with age and the number of cigarettes you smoke.

Birth control pills help to lower the chances of becoming pregnant when taken as directed. They do not protect against HIV infection (AIDS) and other sexually transmitted diseases.

Do not skip any pills, even if you do not have sex often.

If you miss a period, you could be pregnant. However, some women miss periods or have light periods on birth control pills, even when they are not pregnant. Contact your healthcare provider for advice if you:

  • Think you are pregnant
  • Miss one period and have not taken your birth control pills every day
  • Miss two periods in a row

You should stop Lo Loestrin Fe at least four weeks before you have surgery and not restart it until at least two weeks after the surgery, due to an increased risk of blood clots.

If you have vomiting or diarrhea, your birth control pills may not work as well. Use another birth control method, like a condom and spermicide, until you check with your healthcare provider.

If you are scheduled for any laboratory tests, tell your healthcare provider that you are taking birth control pills. Certain blood tests may be affected by birth control pills.

How well does Lo Loestrin Fe work?

Your chance of getting pregnant depends on how well you follow the directions for taking your birth control pills. The better you follow the directions, the less chance you have of getting pregnant.

Based on the results of one clinical study, about 2 to 4 out of 100 women may get pregnant during the first year they use Lo Loestrin Fe.

Women with a BMI above 35 kg/m 2 were not studied in the clinical trial, so it is not known how well this medication protects against pregnancy in such women. If you are overweight, discuss with your healthcare provider whether Lo Loestrin Fe is the best choice for you.

Birth control pills are:

  • Less effective than implants, injections, intrauterine devices and sterilization, which result in fewer than 1 pregnancy per 100 women in a year
  • As effective as a skin patch or vagina ring with hormones
  • Slightly more effective than condoms and diaphragms
  • More effective than using spermicide, withdrawal or having no sex during the most fertile days of the monthly cycle, which result in 10 to 20 pregnancies per 100 women in one year
  • More effective than no birth control, which results in 85 or more pregnancies per 100 women in one year

Who should not take Lo Loestrin FE?

Your healthcare provider will not give you Lo Loestrin Fe if you have:

  • Ever had blood clots in your legs (deep vein thrombosis), lungs (pulmonary embolism), or eyes (retinal thrombosis)
  • Ever had a stroke
  • Ever had a heart attack
  • Certain heart valve problems or heart rhythm abnormalities that can cause blood clots to form in the heart
  • An inherited problem with your blood that makes it clot more than normal
  • High blood pressure that medicine can’t control
  • Diabetes with kidney, eye, nerve, or blood vessel damage
  • Ever had certain kinds of severe migraine headaches with aura, numbness, weakness or changes in vision
  • Ever had breast cancer, which may be sensitive to female hormones
  • Liver disease, including liver tumors
  • Take any Hepatitis C drug combination containing ombitasvir/paritaprevir/ritonavir, with or without dasabuvir. This may increase levels of the liver enzyme alanine aminotransferase (ALT) in the blood.

Also, do not take birth control pills if you:

  • Smoke and are over 35 years old
  • Are allergic to anything in this medication. See below for a complete list of ingredients.

Birth control pills may not be a good choice for you if you have ever had jaundice (yellowing of the skin or eyes) caused by pregnancy, also called cholestasis of pregnancy.

What should I tell my doctor before taking Lo Loestrin FE?

Before you take Lo Loestrin Fe, tell your healthcare provider about all of your medical conditions, including if you have ever had any of the conditions listed above in ‘Who should not take Lo Loestrin Fe?”.

How should I take Lo Loestrin FE?

  • Take one pill every day at the same time. If you miss pills you could get pregnant. This includes starting the pack late. The more pills you miss, the more likely you are to get pregnant.
  • You may have spotting or light bleeding, or may feel sick to your stomach during the first few months of taking this medication. If you have spotting or light bleeding or feel sick to your stomach, do not stop taking the pill. The problem will usually go away. If it doesn’t go away, check with your healthcare provider.
  • Missing pills can also cause spotting or light bleeding, even when you take these missed pills later. On the days you take 2 pills to make up for missed pills, you could also feel a little sick to your stomach.
  • If you have trouble remembering to take this medication, talk to your healthcare provider about how to make pill-taking easier or about using another method of birth control.
  • If you have vomiting or diarrhea (within 3 to 4 hours after you take your pill), you should follow the instructions for “What happens if I miss a dose”.
  • If you have any questions or are unsure about the information in this leaflet, call your healthcare provider.

Before you start taking

1. Decide what time of day you want to take your pill. It is important to take it at the same time every day.

2. Look at your pill pack: The pill pack has 24 “active” blue pills (with hormones) and 2 “active” white pills (with hormones) for Weeks 1, 2, 3 and part of Week 4. The pill pack also has 2 “reminder” brown pills (without hormones) for the last part of Week 4.

  • Where on the pack to start taking pills,
  • In what order to take the pills (follow the arrows), and
  • The week numbers as shown in the picture above

4. Have ready at all times:

  • Another kind of birth control (such as a condom and spermicide) to use as a back-up in case you miss pills
  • An extra pill pack

When to start the first pack

1. Take the day label strip that starts with the first day of your period. (This is the day you start bleeding or spotting, even if it is almost midnight when the bleeding begins.)

2. Place the day label strip on the tablet dispenser over the area that has the days of the week (starting with Sunday) printed on the plastic.

3. Take the first blue pill of the first pack during the first 24 hours of your period.

4. You will not need to use a back-up method of birth control, because you are starting the pill at the beginning of your period. However, if you start this medication later than the first day of your period, or if you start after having a baby and you have not yet resumed your periods, you should use another method of birth control (such as a condom and spermicide) as a back-up method until you have taken 7 blue pills.

When you switch from a different method of hormonal contraception

  • When you switch from another birth control pill, start this medication on the first day you would have started your previous birth control pack.
  • When you switch from a vaginal ring or skin patch, finish the 21 days of use, and wait 7 days after removal of the ring or patch before starting this medication.
  • When you switch from a progestin-only pill, start Lo Loestrin Fe the next day.
  • When you switch from an implant, start Lo Loestrin Fe on the day of implant removal.
  • If you switch from an injectable contraceptive, start Lo Loestrin Fe on the day on which the next injection would be due.
  • If you switch from an IUD, discuss with your healthcare provider when to start Lo Loestrin Fe.

What to do during the month

  • Take one pill at the same time every day until the pack is empty.
    • Do not skip pills even if you are spotting or bleeding between monthly periods or feel sick to your stomach (nausea).
    • Do not skip pills even if you do not have sex very often.

    What should I know about my period when taking?

    When you take Lo Loestrin Fe you may have bleeding and spotting between periods, called unscheduled bleeding. Approximately half of the women who use this medication have unscheduled bleeding or spotting in the first months of use, and about one-third of users continue to have unscheduled bleeding or spotting after one year of use. If the unscheduled bleeding or spotting is heavy or lasts for more than a few days, you should discuss this with your healthcare provider.

    What if I miss my scheduled period when taking?

    It is not uncommon to miss your period. However, if you go two or more months in a row without a period, or you miss your period after a month where you did not take all your pills correctly, call your healthcare provider because you may be pregnant. Also notify your healthcare provider if you have symptoms of pregnancy such as morning sickness or unusual breast tenderness. Stop taking Lo Loestrin Fe if you are pregnant.

    What happens if I miss a dose?

    Lo Loestrin Fe may not be as effective if you miss any blue or white pills, especially if you miss the first few or the last few blue pills in a pack.

    If you miss ONE blue pill, follow these steps:

    • Take it as soon as you remember. Take the next pill at your regular time. This means you may take two pills in 1 day.
    • You do not need to use a back-up birth control method if you have sex.

    If you miss TWO blue pills in a row in Week 1 or Week 2 of your pack, follow these steps:

    • Take:
      • Two pills on the day you remember and two pills the next day.
      • Then one pill a day until you finish the pack.

      If you miss TWO pills (blue or white) in a row in Week 3 or Week 4 of your pack, follow these steps:

      • Throw out the rest of the pill pack and start a new pack that same day.
      • You may not have your period this month but this is expected. However, if you miss your period 2 months in a row, call your healthcare provider because you might be pregnant.
      • You could become pregnant if you have sex during the first 7 days after you restart your pills. You MUST use a non-hormonal birth control method (such as a condom and spermicide) as a back-up for those 7 days after you restart your pills.

      If you miss THREE OR MORE pills (blue or white) in a row at any time, follow these steps:

      • Throw out the rest of the pill pack and start a new pack that same day.
      • You may not have your period this month but this is expected. However, if you miss your period 2 months in a row, call your healthcare provider because you might be pregnant.
      • You could become pregnant if you have sex on the days when you missed pills or during the first 7 days after restarting your pills. You MUST use a non-hormonal birth control method (such as a condom and spermicide) as a back-up the next time you have sex and for the first 7 days after you restart your pills.

      If you forget either of the 2 brown “reminder” pills in Week 4, follow these steps:

      • Throw away the pill you missed.
      • Start a new pack on the same day you usually start a new pack.
      • You do not need to use a back-up method of birth control.

      If you are still not sure what to do about the pills you have missed:

      • Use a back-up method of birth control anytime you have sex.
      • Keep taking one pill each day until you can reach your healthcare provider.

      What happens if I overdose?

      Seek emergency medical attention or call the Poison Help line at 1-800-222-1222. Overdose may cause nausea or vaginal bleeding in females.

      No serious problems have been reported from a birth control pill overdose, even when accidentally taken by children.

      Dosing information

      • Take one tablet by mouth at the same time every day for 28 days.
      • Take tablets in the order directed on the blister pack.

      What are the side effects of Lo Loestrin FE?

      Serious side effects of Lo Loestrin Fe and other birth control pills include:

      • An increase the risk of serious blood clots, especially in women who have other risk factors, such as smoking, obesity, or age greater than 35 (an increased risk of serious blood clots also occurs during pregnancy). It is possible to die from a problem caused by a blood clot, such as a heart attack or a stroke.
        Some examples of serious blood clots are blood clots in the:
        • Legs (deep vein thrombosis)
        • Lungs (pulmonary embolus)
        • Eyes (loss of eyesight)
        • Heart (heart attack)
        • Brain (stroke)

        All of these events are uncommon in healthy women.

        Call your healthcare provider right away if you have:

        • Persistent leg pain
        • Sudden shortness of breath
        • Sudden blindness, partial or complete
        • Severe pain or pressure in your chest
        • Sudden, severe headache unlike your usual headaches
        • Weakness or numbness in an arm or leg, or trouble speaking
        • Yellowing of the skin or eyeballs

        Common side effects of Lo Loestrin Fe and other birth control pills include:

        • Spotting or bleeding between menstrual periods
        • Nausea
        • Breast tenderness
        • Headache

        These side effects are usually mild and usually disappear with time.

        Less common side effects of Lo Loestrin Fe are:

        • Acne
        • Less sexual desire
        • Bloating or fluid retention
        • Blotchy darkening of the skin, especially on the face
        • High blood sugar, especially in women who already have diabetes
        • High fat (cholesterol, triglyceride) levels in the blood
        • Depression, especially if you have had depression in the past. Call your healthcare provider immediately if you have any thoughts of harming yourself
        • Problems tolerating contact lenses
        • Weight gain

        This is not a complete list of possible side effects. Talk to your healthcare provider if you develop any side effects that concern you. You may report side effects to the FDA at 1-800-FDA-1088.

        Do birth control pills cause cancer?

        It is not known if hormonal birth control pills cause breast cancer. Some studies, but not all, suggest that there could be a slight increase in the risk of breast cancer among current users with longer duration of use.

        If you have breast cancer now, or have had it in the past, do not use hormonal birth control because some breast cancers are sensitive to hormones.

        Women who use birth control pills may have a slightly higher chance of getting cervical cancer. However, this may be due to other reasons such as having more sexual partners.

        Interactions

        Tell your healthcare provider about all medicines and herbal products that you take. Some medicines and herbal products may make birth control pills such as Lo Loestrin Fe less effective, including:

        • barbiturates
        • bosentan
        • carbamazepine
        • felbamate
        • griseofulvin
        • oxcarbazepine
        • phenytoin
        • rifampin
        • St. John’s wort
        • topiramate

        Consider using another birth control method when you take medicines that may make birth control pills less effective.

        Birth control pills may interact with lamotrigine, an anticonvulsant used for epilepsy. This may increase the risk of seizures, so your healthcare provider may need to adjust the dose of lamotrigine.

        Pregnancy and breastfeeding

        Birth control pills should not be taken during pregnancy. However, birth control pills taken by accident during pregnancy are not known to cause birth defects.

        If you are breastfeeding, consider another birth control method until you are ready to stop breastfeeding. Birth control pills that contain estrogen, like Lo Loestrin Fe, may decrease the amount of milk you make. A small amount of the pill’s hormones pass into breast milk.

        What if I want to become pregnant?

        You may stop taking Lo Loestrin Fe whenever you wish. Consider a visit with your healthcare provider for a pre-pregnancy checkup before you stop taking the pill.

        What are the ingredients in Lo Loestrin FE?

        Active ingredients: ethinyl estradiol and norethindrone acetate

        Blue tablets: 1 mg norethindrone acetate and 10 mcg ethinyl estradiol

        White tablets: 10 mcg ethinyl estradiol

        Blue tablets: mannitol, microcrystalline cellulose, FD&C Blue No. 1 Aluminum Lake, sodium starch glycolate, magnesium stearate, povidone, vitamin E and lactose monohydrate.

        White tablets: mannitol, microcrystalline cellulose, sodium starch glycolate, magnesium stearate, povidone, vitamin E and lactose monohydrate.

        Brown tablets: ferrous fumarate, mannitol, povidone, microcrystalline cellulose, sodium starch glycolate, magnesium stearate, sucralose and spearmint flavor. The ferrous fumarate tablets do not serve any therapeutic purpose. They do not contain enough iron to treat iron deficiency and are used as a reminder to take a tablet everyday.

        Warning

        • Smoking cigarettes while using Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate) raises the chance of very bad heart and blood-related side effects. This chance is raised with age (mainly in women older than 35 years of age). It is also raised with the number of cigarettes smoked. It is strongly advised not to smoke. Do not use Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate) if you smoke and are older than 35 years of age.

        Uses of Loestrin Fe 1/20:

        • It is used to prevent pregnancy.
        • It is used to treat pimples (acne).
        • It may be given to you for other reasons. Talk with the doctor.

        What do I need to tell my doctor BEFORE I take Loestrin Fe 1/20?

        • If you are allergic to Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate); any part of Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate); or any other drugs, foods, or substances. Tell your doctor about the allergy and what signs you had.
        • If you have had any of these health problems: Blood clots, blood clotting problem, breast cancer or other cancer where hormones make it grow, diseased blood vessels in the brain or heart, disease of a heart valve with problems, heart disease, chest pain caused by angina, heart attack, stroke, high blood pressure, liver disease, liver tumor, very bad headache or migraine, or diabetes that affects blood flow.
        • If you have had any of these health problems: Endometrial cancer, cancer of the cervix or vagina, or vaginal bleeding where the cause is not known.
        • If you are taking ombitasvir, paritaprevir, and ritonavir (with or without dasabuvir).
        • If you are pregnant or may be pregnant. Do not take Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate) if you are pregnant.
        • If you are breast-feeding or plan to breast-feed.
        • If you turned yellow during pregnancy or with estrogen-based or hormone contraceptive use.
        • If your child has not had her first period.

        Tell your doctor and pharmacist about all of your drugs (prescription or OTC, natural products, vitamins) and health problems. You must check to make sure that it is safe for you to take Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate) with all of your drugs and health problems. Do not start, stop, or change the dose of any drug without checking with your doctor.

        What are some things I need to know or do while I take Loestrin Fe 1/20?

        • Tell all of your health care providers that you take Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate). This includes your doctors, nurses, pharmacists, and dentists. This medicine may need to be stopped before certain types of surgery as your doctor has told you. If Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate) is stopped, your doctor will tell you when to start taking Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate) again after your surgery or procedure.
        • This medicine may raise the chance of blood clots, a stroke, or a heart attack. Talk with the doctor.
        • Talk with your doctor if you will need to be still for long periods of time like long trips, bedrest after surgery, or illness. Not moving for long periods may raise your chance of blood clots.
        • Some studies have shown the risk of breast cancer is raised in women taking birth control pills, especially at a younger age. The risk was also linked to how long the birth control pills were taken. One study showed the risk was also raised in women who took birth control pills within the past 10 years.
        • If you have high blood sugar (diabetes), talk with your doctor. This medicine may raise blood sugar.
        • Check your blood sugar as you have been told by your doctor.
        • High blood pressure has happened with drugs like this one. Have your blood pressure checked as you have been told by your doctor.
        • Have blood work checked as you have been told by the doctor. Talk with the doctor.
        • Be sure to have regular breast exams and gynecology check-ups. You will also need to do breast self-exams as you have been told.
        • If you are allergic to tartrazine, talk with your doctor. Some products have tartrazine.
        • If you drink grapefruit juice or eat grapefruit often, talk with your doctor.
        • This medicine may affect certain lab tests. Tell all of your health care providers and lab workers that you take Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate).
        • Certain drugs, herbal products, or health problems could cause Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate) to not work as well. Be sure your doctor knows about all of your drugs and health problems.
        • This medicine does not stop the spread of diseases like HIV or hepatitis that are passed through having sex. Do not have any kind of sex without using a latex or polyurethane condom. If you have questions, talk with your doctor.
        • This medicine may raise the chance of blood clots. The chance is highest during the first year of using Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate). The chance is also highest when starting to use hormone-based birth control again after not using it for 4 weeks or more. Talk with the doctor.
        • If you are taking Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate) for pimples (acne), you need to be at least 15 years of age.
        • If you have any signs of pregnancy or if you have a positive pregnancy test, call your doctor right away.

        How is this medicine (Loestrin Fe 1/20) best taken?

        Use Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate) as ordered by your doctor. Read all information given to you. Follow all instructions closely.

        • Take Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate) at the same time of day.
        • Take with or without food. Take with food if it causes an upset stomach.
        • If you also take colesevelam, take it at least 4 hours before or after you take Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate).
        • After starting Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate), you may need to use a non-hormone type of birth control like condoms to prevent pregnancy for some time. Follow what your doctor has told you to do about using a non-hormone type of birth control.
        • Do not skip doses, even if you do not have sex very often.
        • If you throw up or have diarrhea, Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate) may not work as well to prevent pregnancy. Use an extra form of birth control, like condoms, until you check with your doctor.
        • If you miss 2 periods in a row, take a pregnancy test before starting a new cycle.

        What do I do if I miss a dose?

        • If a dose is missed, check the package insert or call the doctor to find out what to do. If using Loestrin Fe 1/20 (ethinyl estradiol, norethindrone, and ferrous fumarate) to prevent pregnancy, another form of birth control may need to be used for some time to prevent pregnancy.

        What are some side effects that I need to call my doctor about right away?

        WARNING/CAUTION: Even though it may be rare, some people may have very bad and sometimes deadly side effects when taking a drug. Tell your doctor or get medical help right away if you have any of the following signs or symptoms that may be related to a very bad side effect:

        • Signs of an allergic reaction, like rash; hives; itching; red, swollen, blistered, or peeling skin with or without fever; wheezing; tightness in the chest or throat; trouble breathing, swallowing, or talking; unusual hoarseness; or swelling of the mouth, face, lips, tongue, or throat.
        • Signs of liver problems like dark urine, feeling tired, not hungry, upset stomach or stomach pain, light-colored stools, throwing up, or yellow skin or eyes.
        • Signs of high blood pressure like very bad headache or dizziness, passing out, or change in eyesight.
        • Signs of gallbladder problems like pain in the upper right belly area, right shoulder area, or between the shoulder blades; yellow skin or eyes; fever with chills; bloating; or very upset stomach or throwing up.
        • Weakness on 1 side of the body, trouble speaking or thinking, change in balance, drooping on one side of the face, or blurred eyesight.
        • Depression or other mood changes.
        • Swelling.

        What are some other side effects of Loestrin Fe 1/20?

        All drugs may cause side effects. However, many people have no side effects or only have minor side effects. Call your doctor or get medical help if any of these side effects or any other side effects bother you or do not go away:

        • Feeling more or less hungry.
        • Weight gain or loss.
        • Dizziness or headache.
        • Upset stomach or throwing up.
        • Stomach cramps.
        • Bloating.
        • Enlarged breasts.
        • Tender breasts.
        • This medicine may cause dark patches of skin on your face. Avoid sun, sunlamps, and tanning beds. Use sunscreen and wear clothing and eyewear that protects you from the sun.
        • Period (menstrual) changes. These include lots of bleeding, spotting, or bleeding between cycles.
